Triage & 1-2-1 Project Worker

Job description:

Working alongside the HOME Programme Coordinatorand delivery team, this role will be at the forefront of receiving referralsthat come into HOME.

The Triage & 1-2-1 Project Worker will beresponsible for following the day-to-day HOME referral process, includinglogging all new referrals on Lamplight and responding to them in a timelyfashion.

They will offer initial meetings to young peopleand parents to ascertain whether HOME is an appropriate service for them and ifso, which programme they would be most suitable for.

They will also ensure that young people,parents, carers and professionals receive a range of signposting informationregarding the wider support available in Barnsley.

Where necessary, this post will work with youngpeople on a 1-2-1 basis if, where we consider they need some short-term supportto be able to access HOME’s wellbeing group programme or external services.

The right candidate will need to be extremelyorganised, be able to manage their time effectively and have excellentadministrative and IT skills.

This role is subject to a satisfactory EnhancedDBS check and references.
