An exciting opportunity to work with our Warm Connections team in the North Area Council, providing home assessments, personal outcomes planning and review to people experiencing social isolation and/or cold homes.
We require an energetic and enthusiastic person who has at least 2 years' experience of working in a Social Care or Community Development setting
The ability and experience of assessing and creating person centred plans to meet individual needs is essential. You will also have experience of managing risk and an understanding of issues affecting lone workers in peoples homes.
You will also need to be an excellent communicator and have the ability to network and develop partnerships. You will need to manage a complex workload, prioritising effectively where necessary including an ability to work independently without direct supervision.
You must be IT literate and be able to produce reports and documents accurately.
